Hlavaty Dental Arts is a Fee-For-Service practice, which means we can offer you the care you need at a fair price, without being restricted by the insurance companies. As a courtesy to our patients with insurance, we will gladly submit your claims (with all required information, forms, & x-rays) as an out-of-network provider & your insurance company will send your reimbursement checks directly to you at home.
Ask our office about other ways to pay that may be available to you, including healthcare lenders or other third-party financing options.
For those who don't have dental insurance, the best way to get your yearly exams, x-rays, & cleanings covered and save significant money on dental services is through our Hlavaty Dental Arts Membership Plan. Members receive free twice-yearly preventive exams with cleanings and x-rays, as well as discounts on
ALL additional dental services (with NO yearly maximum).
